Well, NGIO still works in this case, but not as intended, as with the given configuration no grass cache is ever generated, even dynamically, so caching can't be used - per my own experience. In the case of a user who wishes to skip grass LOD generation, this discrepancy produces a non-working NGIO setup because the precaching procedure is never described and is never done.
